NUMA innovates in healthcare processes by adding a new local agent to social services, pharmacies, as an actor of proximity to improve the relationship with patients, the monitoring of their conditions and the prevention of frailty. The project will work with ICT tools, using remote management technology and the pharmacy offices as intermediaries between the patient and the health services. A pilot experience will be carried out that allows the evaluation and comparative study of the innovative service in the cross-border area (Andalusia-Algarve), thus improving cooperation between citizens and institutions of both countries.
